coupon for getting your shot. CVS does take most insurances (HMSA, UHA, HMAA,
etc.) but does not accept private insurance like Kaiser or Tricare. If you
elect to take the flu shot and your insurance is not accepted by CVS, there is
an out-of-pocket cost on the day of the clinic. The flu vaccine is $40.99, the
high dose flu (for individuals 65 years and older) is $66.99. Exact cash (no change will be given) or check payable to
Longs Drugs will be accepted. There is no cost if your insurance carrier is accepted by CVS.
Please turn in the completed CVS flu shot authorization form and a copy of your insurance card to Kristina as soon as possible to schedule your time slot.
